School Physicals

School physicals are offered throughout the year, including every day, holiday, and weekend. To streamline your child's school physical, ensure that all required paperwork is completed. Please click on the link labeled "FLORIDA SCHOOL ENTRY EXAM" to access the DH3040-CHP-07/2013 form. Parents or guardians are required to fill out page 1 with the child's medical history and sign and date the form.
