For more than 45 years, Health Resource Services Administration (HRSA) supported health centers have provided comprehensive, culturally competent, quality primary health care services to medically underserved communities and vulnerable populations. Health centers are community-based and patient-directed organizations that serve populations with limited access to health care. We continue to evolve Family Healthcare according to the model of the Patient Centered Medical Home (PCMH) which puts patients at the center of the healthcare system, and provides primary care that is accessible, affordable, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective. We accept Medicaid, Medicare, and insurance plans. Services for the low-income, uninsured are provided on a Sliding Fee Scale that is based on family income and size. As the health care home for over 10,000 residents in Southern Utah, Family Healthcare provides a comprehensive range of health, educational and preventive social services while treating the whole person in a quality, cost efficient and caring environment. We not only offer primary medical care services, but also supportive services that include behavioral health services, patient and family education, family planning, womens health services, and translation services. We believe that each person deserves access to outstanding, state-of-the-art health care and highly skilled health care providers, and we are able to do this through generous federal and state grants; donations from private donors such as Intermountain Healthcare and the Eccles Foundation; and the dedication of all our employees. As the health care home for 13,128 residents in Southern Utah, Family Healthcare provides a comprehensive range of health, educational and preventive social services while treating the whole person in a quality, cost efficient and caring environment. Happy Factory

Miracles happen when people willingly serve others. When Charles and Donna Cooley became aware that many children have never had a toy, they formulated their motto, We may not be able to make a toy for every child in the world that needs onebut were going to try! The seeds of their service were planted in a small workshop at their home near Cedar City, Utah in 1995. They made a couple of hundred toys that were humbly offered to Primary Childrens Medical Center. The toys were received with such enthusiasm and gratitude that the Cooleys made more and donated them locally to the Canyon Creek Womens Crisis Center, Cedar City Care, and Share, the Presbyterian Church, and within the state of Utah to Shriners Hospital, Ronald McDonald House and worldwide. They named their workshop The Happy Factory because of the happiness it brings to them and to the children who receive the wooden toys. In the process, they have learned that toys are not simply playthings, but tools that help unlock a childs ability to think and to cope with the world around them. What started as a hobby has turned into a full-time labor of love.

Turn Community Services

Formerly known as Project TURN, we were founded in 1973 by a group of parents who wanted to improve program services for their adult children with intellectual and developmental disabilities. TURN began by opening residential programs in Ogden and Bountiful where individuals with disabilities could live in a regular home setting and participate in community activities and have an integrated, more independent social interaction experience. Today, more than forty years later, TURN Community Services now provides services to more than 850 individuals with developmental disabilities in Utah. We provide services across the state from Brigham City to St. George, UT. We offer a full range of services including day programs, residential living services, art center programs, summer camp, and supported employment opportunities.

Canyon Creek Services (CCS) is 25 years old and has a powerful legacy involving many amazing individuals and organizations that have sacrificed and built the organization into what it is today and what it will become.Canyon Creek has typically been identified as an emergency shelter for women and children fleeing domestic violence.While that was true in our early years, we have become much more than that and have seen significant expansion in our services and activities over the past few years. Our organization provides free and strictly confidential services to any victim of domestic violence as well as sexual assault. We offer our services to any victim, regardless of age, gender identity, ethnicity, sexual orientation, and so on whether they are in current crisis or seeking other support and guidance.Our current services include 24 hour crisis intervention, a 24 hour hotline, sexual assault hospital response, emotional support and advocacy, legal advocacy, medical advocacy, systems advocacy, housing assistance, emergency shelter, support groups, classes, case management, programming, and other life-situation support and advocacy with the goal of helping survivors achieve emotional healing and financial independence.Importantly, CCS is also providing and expanding sexual assault and domestic violence prevention and education programming, presentations, and materials to individuals, groups, organizations throughout our service area.
